Dimer Health launches AiME, a personalized AI health agent that gives patients real-time rehabilitation guidance at home after discharge, with the option to escalate to a licensed clinician at any time. Based on patients’ discharge instructions and medical records, AiME supports medication management, image sharing, nutrition and exercise plans, and appointment scheduling. Since launching in late 2025, it has supported thousands of patient conversations. Dimer’s transitional care system has reduced 30-day readmission rates by up to 50% among specific patient groups.
